Medical Records Department (MRD)

The Medical Records Department (MRD) is responsible for maintaining and issuing patient medical records in accordance with hospital policies and NABH guidelines.

Location: 3rd Floor – MRD Room Timings: 3:00 PM to 5:00 PM only

Services Provided:

  • Issue of discharge summaries
  • Copies of laboratory and diagnostic reports
  • Certified copies of medical records (on request)
  • Birth & Death Certificate assistance
  • Record verification and documentation support

Important Guidelines:

  • Valid government photo ID proof is mandatory.
  • An authorization letter is required if records are collected by someone other than the patient.
  • Processing time may vary depending on the type of request.
  • All records are handled with strict confidentiality.

Birth & Death Certificate Services

Noor Hospital facilitates the processing and collection of Birth and Death Certificates as per statutory and regulatory requirements.

Timing: 3:00 PM to 5:00 PM only Location: MRD Room – 3rd Floor

Requirements:

  • Valid ID proof
  • Relevant hospital documentation
  • Completed application form (if applicable)

Certificates are issued as per applicable government timelines and hospital policy.

Admission Process

Our admission process is structured to ensure efficiency, clarity, and compliance with quality standards.

Required at the Time of Admission:

  • Valid government-issued ID proof
  • Complete patient and contact details
  • Advance payment as per hospital policy
  • Signed consent forms

Patients are advised to report at least 30 minutes prior to scheduled admission time for smooth processing.

Discharge Process

Discharge is initiated after approval from the treating consultant.

Discharge Procedure Includes:

  • Preparation of discharge summary
  • Finalization of hospital bill
  • Clearance of all outstanding payments
  • Issuance of prescriptions and follow-up instructions

Patients and attendants are requested to carefully review all documents before leaving the hospital premises.

Laboratory & Diagnostic Reports

Our NABL-accredited laboratory ensures high standards of accuracy and reliability in diagnostic services.

Report Collection:

  • Reports can be collected from the respective department.
  • Certain specialized investigations may require additional processing time.
  • Please retain your receipt while collecting reports.

Billing & Payment Information

  • All payments must be made at the hospital billing counter.
  • The hospital currently does not provide cashless insurance facility.
  • All dues must be cleared prior to discharge.
  • Detailed billing statements are available upon request.
